
An anatomical study involving MediCampus Europeo and the Faculty of Medicine of Comenius University Bratislava investigated the nerve supply of the vermiform appendix.
The study, entitled “Morphology of the Vermiform Appendix Innervation”, identified a consistent origin of appendiceal innervation but considerable variability in the way the nerves were distributed near the appendix.
The researchers examined 20 unembalmed adult cadavers, including 17 males and 3 females aged between 30 and 86 years. None of the specimens showed macroscopic abdominal pathology.
Bilateral dissections were performed through a midline abdominal approach. The researchers exposed the superior mesenteric plexus and traced the lumbar splanchnic nerves, preaortic plexuses and sympathetic chains.
They also documented the relationship between the autonomic nerve branches, the ileocolic artery, the appendicular artery and the mesoappendix. Ganglia identified during dissection were removed and histologically verified using haematoxylin and eosin staining.
In all specimens, the nerves supplying the vermiform appendix originated from the superior mesenteric plexus.
The peripheral distribution of these nerves, however, varied considerably.
In some specimens, dense anastomotic neural networks formed close to the appendix. In others, the nerves remained as more isolated perivascular trunks closely following the appendicular artery.
The observed patterns appeared to correspond with variations in the organisation of the ganglia within the superior mesenteric plexus and with differences in the vascular anatomy of the mesoappendix.
Although the embryological basis of appendiceal innervation is consistent, the authors concluded that its peripheral morphology is highly variable.
This variability may help explain atypical pain patterns, differences in pain localisation and the diverse clinical presentations associated with appendiceal disease.
The findings underline the importance of detailed neuroanatomical knowledge when interpreting the relationship between appendiceal anatomy, nerve distribution and clinical symptoms.
